Job summary

CenClear is seeking a compassionate Licensed Practical Nurse (LPN) to support behavioral health clients at our Clearfield and Philipsburg sites with occasional travel to other CenClear locations. The role is full-time, hourly, non-exempt, with four 10-hour days currently scheduled.

You will work closely with psychiatrists and care teams to coordinate care, provide wellness coaching, and ensure accurate documentation. Competitive benefits and growth opportunities are offered.

Qualifications

  • LPN licensed and eligible to obtain required clearances before hire.
  • Minimum 2 years of experience in mental health and substance use services.
  • Strong communication and interpersonal skills.

Responsibilities

  • Work 40 hours weekly, with coverage at Clearfield and Philipsburg locations.
  • Collaborate with prescribers and assist with labs, reports, and vital signs.
  • Provide wellness coaching and related staff training.
  • Document wellness activities and outcomes data.
  • Complete paperwork accurately and on time.
  • Attend monthly supervision meetings.

About the Role:

CenClear's Behavioral Health Division is seeking a compassionate and committed Licensed Practical Nurse (LPN) to join our team. This is a rewarding opportunity to support the health and well-being of individuals in our care and make a lasting difference in their lives.

What You'll Do:
  • Work 40 hours per week, 52 weeks per year
  • Primary coverage required at both Clearfield and Philipsburg locations.
  • Current schedule is four 10-hour days (subject to change based on program needs)
  • Occasional travel to other CenClear locations for additional coverage as needed.
  • Participate in all required CenClear, CCBHC and wellness/health related education and trainings
  • Provide wellness coaching and related training to staff, individuals in services and community partners
  • Promote a culture of health and wellness through ongoing participation in meetings and community outreach
  • Collaborate and coordinate with healthcare providers for continuity of care
  • Facilitate referrals, as needed
  • Collect required data for outcomes measurement and contract requirements and document appropriately
  • Submit all necessary administrative paperwork according to procedures
  • Document all wellness coaching sessions and other wellness/integration activities
  • Assist psychiatric prescribers in providing treatment to clients, collecting labs, reports, information, and vitals for provider
  • Follow up with clients per prescriber request
  • Willingness to learn and adapt to new information and skills and engage in ongoing education, training, and meetings.
  • Self-motivation and ability to work independently as well as a team.
  • Empathy and understanding in dealing with clients and families.
  • Update client history and changes at each appointment.
  • Maintain professionalism and confidentiality
  • Complete paperwork accurately and in a timely manner.
  • Attend and participate in monthly supervisions.

  • Licensed Practical Nurse (LPN)

  • Ability to gather and analyze health-related information.
  • Must obtain the following clearances prior to hire: PA State Police Criminal History, PA Child Abuse History, FBI Criminal History, and Mandated Reporting Certificate
