Job summary

Signature HealthCARE in Clarksville seeks a licensed practical nurse (LPN) to deliver direct nursing care, administer medications, and support residents with daily living activities. The role includes supervising nursing assistants, documenting care, and coordinating admissions, transfers, and discharges with the interdisciplinary team.

Ideal candidates hold an active LPN/LVN license, have 1+ year of clinical nursing experience (long-term care preferred), and are familiar with EHR systems.

Qualifications

• Active LPN/LVN license in the state of practice.
• 1+ year of clinical nursing experience; long-term care preferred.
• Experience in a supervisory or charge nurse role preferred.
• Familiarity with electronic health records (EHR) systems.
• Strong observation, medication administration, and documentation skills.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Supervise and direct nursing assistants and medication aides during the shift.
• Monitor residents’ status and notify physician of changes.
• Develop and review individualized resident care plans with the interdisciplinary team.
• Complete accurate clinical documentation per facility policy and procedure.
• Coordinate resident admissions, transfers, and discharges.

How you Will make a difference

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations, as applicable.
• Monitor and observe residents’ physical and emotional status; notify the attending physician and responsible party of any change in condition.
• Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy and procedure.
• Coordinate resident admissions, transfers, and discharges according to established procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys as needed.
• Perform other duties as assigned by the Director of Nursing or designee.

What you Need to make a Difference

• Current, active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one (1) year of clinical nursing experience; long-term care experience, preferred.
• Experience in a supervisory or charge nurse role, preferred.
• Strong clinical observation, medication administration, and documentation skills.
• Knowledge of Federal and State long-term care regulations.
• Effective verbal and written communication skills.
• Ability to work independently, prioritize tasks, and exercise sound clinical judgment.
• Experience in a supervisory or charge nurse role.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.
