Job summary

Signature HealthCARE is seeking an LPN/LVN to provide direct nursing care to residents in Putnam County. You will administer medications, assist with ADLs, and work under the supervision of the Director of Nursing to ensure safety and quality care.

Ideal candidates have an active LPN/LVN license, CPR/BLS preferred, at least 1 year of clinical nursing experience, and familiarity with EHR systems. Join a team focused on compassionate, resident-centered care in a supportive environment.

Qualifications

  • Active LPN/LVN license in the state of practice.
  • CPR/BLS certification preferred.
  • Minimum one year of clinical nursing experience; long-term care experience preferred.
  • Experience in a supervisory or charge nurse role preferred.
  • Strong clinical observation, medication administration, and documentation skills.
  • Knowledge of Federal and State long-term care regulations.
  • Effective verbal and written communication skills.
  • Familiarity with electronic health records (EHR) systems.
  • Proficiency with Microsoft Office applications.

Responsibilities

  • Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
  • Assist residents with activities of daily living (ADLs) while ensuring comfort, dignity, and safety.
  • Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations.
  • Monitor and observe residents’ physical and emotional status; notify the attending physician and responsible party of any change in condition.
  • Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
  • Complete accurate and timely clinical documentation per facility policy and procedure.
  • Coordinate resident admissions, transfers, and discharges according to established procedures.
  • Participate in quality assurance activities, staff education, and regulatory surveys as needed.
  • Perform other duties as assigned by the Director of Nursing or designee.

How you Will make a difference

  • Ability to work independently, prioritize tasks, and exercise sound clinical judgment.
