Qualifications

• Must be willing to submit to a background check.
• Must have a valid driver license, reliable transportation and auto insurance.
• Must have an unencumbered LPN License in the state you are applying to.
• Must be willing to travel up to a 45 mile radius to patients' homes.
• Must hold an active CPR Certification.

Responsibilities

• Manage home care as assigned by the Case Manager under the Plan of Care.
• Able to bend and stand an average of 6 hours per day.
• Lifts up to 50-75 pounds as required by patient needs.
• Maintain timely and accurate documentation.
• Provide medication management and teaching as directed by the Plan of Care.
• Collaborate with the care team to ensure optimal patient outcomes.
