Job summary

Aliya Healthcare in Chicago is seeking a dedicated Licensed Practical Nurse (LPN) to join our compassionate care team at our skilled nursing facility.

As an LPN, you will provide total nursing care to residents, participate in care planning, administer medications, and supervise team members while ensuring adherence to facility standards.

Qualifications

• Graduation from an accredited nursing education program.
• Active LPN license in Illinois.
• No prior abuse convictions.
• Strong communication and teamwork skills.
• Ability to follow policies, maintain safety, and deliver empathetic care.
• Ability to work with diverse resident populations, including elderly and mentally ill individuals.
• Physical ability to perform nursing duties with or without accommodations.

Responsibilities

• Provide total nursing care to residents on the assigned unit during the shift as an LPN.
• Participate in developing and implementing individualized care plans with the healthcare team.
• Administer medications and treatments as prescribed and evaluate effectiveness.
• Maintain an environment that promotes residents’ physical, social, and psychological well-being.
• Educate residents and families on care plans and medical procedures.
• Monitor team member performance and assist with development, evaluation, and corrective action.
• Delegate and ensure completion of assigned nursing tasks to team members.
• Stay current with facility policies, procedures, and ongoing nursing education.
• Recognize and act on significant changes in resident conditions.
• Document care, responses, and relevant information per facility guidelines.
• Act as a charge nurse: answer questions, resolve issues, and support resident care decisions.
• Follow all facility safety policies and procedures
