Job summary

Chestertown Nursing and Rehab is seeking a Licensed Practical Nurse (LPN) to support residents' health and daily comfort. You will provide hands-on care, work with residents, families, and the care team to promote dignity and quality of life.

Responsibilities include direct supervision of geriatric nursing assistants, following established policies, preparing and administering medications, assessing residents, and coordinating admissions, discharges, and transfers.

Qualifications

• Must hold a nursing degree from an accredited college or university.
• Current unrestricted LPN license in practicing state.
• CPR certification required.
• Direct the daily activities of geriatric nursing assistants per regulations.
• Follow the nursing model to promote holistic care.

Responsibilities

• Participates in admission, discharge, and transfers as required.
• Assesses changes in residents' status and notifies physicians and families.
• Transcribes physician orders to medical records and carries out orders as written.
• Prepares and administers medications as ordered by physicians and monitors for adverse effects.
• Performs wound treatments as ordered and documents findings.
• Coordinates with residents, families, and care team to ensure quality of life.
