Three Rivers Care in Centralia, WA is seeking a Licensed Practical Nurse (LPN) for immediate hire. The role involves medication administration, patient monitoring, and coordinating with the care team to ensure high-quality patient outcomes.

• Current and valid LPN license.

Responsibilities

• Observe, administer, record medications, treatments, and patient's vital signs, symptoms, and general condition.
• Reinforce support and provide health care instructions to patients and their families.
• Participates with other team members in planning, implementing and evaluating nursing care and other patient activities.
• Assist with admission, transfer, and discharge of patients.
• Assist with unit management and/or assume charge of unit as assigned.
