A Licensed Practical Nurse is responsible for carrying out patient care in accordance with physician's orders under the guidance and direction of the Registered Nurse in charge and supervision of the Team Manager.

Essential Duties and Responsibilities a Licensed Practical Nurse:

  • Provides skilled nursing care to patients as directed by the IDG plan of care in accordance with agency policies. This includes personal care, vital signs, and administration of medications, treatments and any other procedures within their professional practice following the policies of the agency.
  • Documents to agency standards, timely completion, timely signatures, using A-I-R protocol.
  • Reinforces patient and family teaching that may include information on the disease process, self-care techniques, and prevention strategies under the guidance of registered nurse assigned to the same case.
  • Helps to achieve and maintain continuity of patient care by assisting in planning and exchanging information with other health professionals.
  • Assists the physician and registered nurse in performing specialized procedures.
  • Elicit patient/family complaints or unusual occurrences (i.e., reportable incidents).
  • Demonstrates proper sharps disposal, hand hygiene; waste disposal; use of personal protective equipment.
  • Communicates current health status and needs of patients to appropriate health professionals, contributing to the coordination of the total plan of care inclusive of review/revision of the nursing care plan, evaluation of the effectiveness and attainment of patient care outcomes, and discharge planning.
  • Participates in the quality improvement program and documents quality assurance data as required.
  • Participates in weekend, holiday rotation and back up visits as needed.
  • Adheres to requirements of the organizations policies and procedures that are both patient focused (clinical) and organizational focused (administrative).
  • Adheres to HIPPA regulations and policies. Maintains the security of patient electronic/written records.

Qualifications for a Licensed Practical Nurse:

  • Education/Certification
  • Graduate from an accredited School of Practical Nursing with current LPN licensure in the State of New York.
  • Valid Driver's License, free of serious violations, and vehicle required for use when on duty or on-call.
  • Experience
  • 1 year of nursing practice as a Licensed Practical Nurse preferred.
