Job summary

Bellbrook Health and Rehab in Bellbrook, OH is hiring a Licensed Practical Nurse (LPN) for full-time and part-time schedules. A $3,000 sign-on bonus is offered.

The LPN will provide direct nursing care under federal, state, and local guidelines and supervise CNAs to ensure safe, quality resident care. Responsibilities include supervising CNA staff, administering medications, coordinating admissions and transfers, and ensuring compliance with facility policies.

Qualifications

• Licensed Practical Nurse with current state license.
• Current CPR certification.
• IV certification, if applicable.
• Experience in long-term care preferred but not required.

Responsibilities

• Directs the daily activities of certified nursing assistants in accordance with regulations and facility policies.
• Follows the nursing model to promote holistic care for residents.
• Ensures compliance with policies and procedures by nursing staff.
• Participates in admissions, discharges and transfers of residents as required.
• Assesses changes in residents’ status and communicates with physician and family.
• Transcribes physician orders and carries them out as written.
• Prepares and administers medications as ordered and monitors for adverse effects.
• Performs wound treatments and documents changes.
• Schedules follow-up appointments and transportation as needed.
• Provides nursing leadership to the unit staff.

Position Summary

The Licensed Practical Nurse (LPN) may provide direct nursing care to the residents and supervises the day-to-day nursing activities performed by certified nursing assistants in accordance with current federal, state, and local regulations and guidelines and established facility policies and procedures.

Key Responsibilities

• Directs the daily activities of the certified nursing assistants in accordance with current federal, state and local regulations and guidelines and established facility policies and procedures.
• Follows the nursing model to promote holistic care for the residents.
• Ensures that policies and procedures are complied with by nursing personnel assigned.
• Participates in the admission, discharge and transfers of residents as required.
• Assesses for changes in residents’ status, notifying the physician and resident’s family or representative and documenting accordingly.
• Transcribes physician orders to medical record and carries out orders as written.
• Prepares and administers medications as per physicians’ orders and observes for adverse effects as indicated.
• Performs wound treatments as per physicians’ orders, observes for changes and documents accordingly as indicated.
• Schedules follow up appointments for residents and transportation needs as indicated.
• Provides nursing leadership to nursing personnel assigned to the unit.
• Other duties as assigned.

Qualifications

• Prior experience in long-term care preferred but not required.
• A Nursing degree from an accredited college or university.
• Current unrestricted license as a Licensed Practical Nurse (LPN) in practicing state.
• Current CPR certification.
• IV certification, if applicable
