Job summary

Centerville Health and Rehab in Centerville, OH seeks a Licensed Practical Nurse (LPN) for PRN shifts to provide direct resident care and supervise CNAs in line with policies and regulations.

The ideal candidate will hold an LPN license, CPR and IV certification, have a nursing degree, and be able to perform med administration, wound care, and coordinate admissions, discharges and transfers while delivering compassionate, outcome-oriented care.

Qualifications

• LPN licensure in practicing state.

Responsibilities

• Directs the daily activities of certified nursing assistants.
• Promotes holistic care for residents.

• Assesses changes in residents' status and notifies physician/family.
• Transcribes orders and administers medications.
• Performs wound care as ordered and documents.
• Schedules follow-up appointments and transportation as needed.
• Provides nursing leadership to unit staff.

Position Summary

The Licensed Practical Nurse (LPN) may provide direct nursing care to the residents and supervises the day-to-day nursing activities performed by certified nursing assistants in accordance with current federal, state, and local regulations and guidelines and established facility policies and procedures.

Key Responsibilities

• Directs the daily activities of the certified nursing assistants in accordance with current federal, state and local regulations and guidelines and established facility policies and procedures.
• Follows the nursing model to promote holistic care for the residents.
• Ensures that policies and procedures are complied with by nursing personnel assigned.
• Participates in the admission, discharge and transfers of residents as required.
• Assesses for changes in residents’ status, notifying the physician and resident’s family or representative and documenting accordingly.
• Transcribes physician orders to medical record and carries out orders as written.
• Prepares and administers medications as per physicians’ orders and observes for adverse effects as indicated.
• Performs wound treatments as per physicians’ orders, observes for changes and documents accordingly as indicated.
• Schedules follow up appointments for residents and transportation needs as indicated.
• Provides nursing leadership to nursing personnel assigned to the unit.
• Other duties as assigned.

Qualifications

• Prior experience in long-term care preferred but not required.
• A Nursing degree from an accredited college or university.
• Current unrestricted license as a Licensed Practical Nurse (LPN) in practicing state.
• Current CPR certification.
• IV certification, if applicable
