Job summary

Interim HealthCare Inc. is seeking a Licensed Practical Nurse (LPN) for in-home care in Mann's Choice. The role offers true flexibility with full-time, part-time, and PRN options, and shift schedules from 5AM–3PM or 10PM–10AM.

You will provide direct patient care in the home setting, following a prescribed plan of care and nursing scope of practice, with a focus on comfort and dignity. The position requires an accredited nursing program graduate with an active LPN license in the relevant state,

Qualifications

• Must have graduated from an accredited Nursing program.
• Current unrestricted LPN license to practice in the state for this position.
• Current CPR/AED/BLS/First Aid certification.
• Reliable transportation to/from care sites.
• Trach/ventilatory experience is preferred but not required.

Responsibilities

• Provide in-home direct patient care according to prescribed plan and nursing scope of practice.
• Assess and document patient conditions and report changes as needed.
• Administer medications, insulin, and IV fluids and document thoroughly.
• Inspect and care for wounds, change dressings, assist with hygiene.
• Assess for further injuries or infections and deliver appropriate treatments.
• Collaborate with providers and other health professionals to ensure patient outcomes.

As a Licensed Practical Nurses (LPN) you will:

• Providein-homedirect patient care according to theprovider'sprescribed plan of care (and nursingscope of practice) while closely monitoring progress/recovery
• Assessand document patient conditions and reportchangeas needed (vitals,blood/stool/urine, glucose,etc.)
• Administer medication, insulin, and IV/fluids,documenting thoroughly

• Assess patient for further injuries, infections, bedsores, etc. and deliver appropriate treatments
• Collaborate with providers, aides, and allied health professionals to ensure exceptional patient outcomes

• Educational: Graduate of an accredited Nursing program
• Licensure: Current unrestricted license to practice as Licensed Practical Nurse (LPN) in the state associated with this position
• Current CPR/AED/BLS/First Aid certification
• Reliable transportation to/from care sites and/or work locations
• Practical trachand/or ventilator experience preferred, not required
