Job Title: Outbound Call Center Representative (LPN/LVN)

Job Type: Contract to Hire

Location: Remote

Hours: 9 AM - 6 PM EST (Monday - Friday)

Rate: Starting at $25.00/hour

Responsibilities

• Outbound Calls: Make outreach calls to patients scheduled for home visits and telehealth appointments to confirm their appointments and provide necessary information.
• Chart Preparation: Initiate chart preparation by gathering and verifying patient information to assist healthcare providers with upcoming appointments.
• Data Entry: Accurately enter patient data into the Electronic Medical Records (EMR) system.
• Patient Assessments: Conduct assessments with patients over the phone to gather health information and address any concerns.
• Follow-Up: Perform follow-up calls to ensure patients have completed any required pre-appointment tasks and are ready for their visits.
• Documentation: Maintain detailed records of patient interactions and update EMR accordingly.
• Coordination: Coordinate with healthcare providers and other team members to ensure seamless patient care.
• Problem-Solving: Address any issues or questions patients may have regarding their appointments or health concerns.
• Compliance: Adhere to all healthcare regulations and company policies during patient interactions.
• Continuous Improvement: Participate in training sessions and team meetings to stay updated on best practices and improve service quality.
  
  

Required Qualifications

• Must hold a current Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) license.
• Proficiency in Microsoft Office applications (Word, Excel, Outlook).
• Experience with eClinicalWorks (eCW) or similar EMR systems is preferred.
