ClearSky Rehabilitation Hospital of Avondale, AZ is seeking a Licensed Practical Nurse (LPN) to join our compassionate care team. You will provide high-quality, patient-centered care in a hospital-like setting with a focus on recovery from complex injuries and illnesses.

About the Role:

This position offers a higher level of clinical care than typically found in post-acute or residential settings. Our facility delivers hospital-level services that are comparable to those provided on a medical-surgical unit. Our highly skilled interdisciplinary team is equipped to manage a wide range of complex medical needs, including intravenous therapy, surgical and wound care, tracheostomy management, mobility assistance, and the comprehensive oversight of lines, tubes, and drains. As a Licensed Practical Nurse (LPN), you will play a critical role in delivering high-quality, patient-centered care within a healing environment focused on optimal recovery.

Position Highlights at a Glance:

• Caseload: ~7 patients
• Average Patient Length of Stay: 10–14 days
• Conditions: Stroke, brain & spinal cord injuries, cardiac, ortho, complex medical, and more
• Team Approach: Work closely with PT/OT/ST, physicians, case managers, respiratory therapy & more
• Advanced Care: IVs, trachs, wound care, mobility support, lines, tubes, and drains
• 12-hour shifts, 3 days/week + rotating weekends/holidays (with added pay)

What You’ll Need:

• LPN License (State of AZ)
• CPR required | ACLS & BLS preferred
• 1+ year of LPN experience preferred (Acute care a plus)

Physical Requirements Over the Course of a Shift:

• A significant amount of standing, walking, bending, lifting, reaching, pushing, and pulling, often for prolonged periods of time.
• Both gross and precise motor functions.
• Lifting/exerting of up to 50 lbs.
• Possible exposure to bodily fluids.
• Visual acuity required for patient assessment and documentation of care.
• Acute hearing required for accurate patient assessment.
• Prolonged periods of standing, bending, and reaching.
• Sufficient manual dexterity to operate equipment and computer keyboard.
