Job summary

Cedar Crest Post Acute is seeking an LPN to provide compassionate, high-quality nursing care in a 166-bed Sub-Acute and Long-Term Care facility. The role emphasizes resident well-being, adherence to care plans, and collaboration with an interdisciplinary team.

Qualifications

• Current, unencumbered PA LPN license.
• Valid BLS Certification (CPR) required.
• Ability to care for and manage multiple patients in a fast-paced healthcare environment.
• Strong clinical judgment, communication skills, and attention to detail.

Responsibilities

• Provide patient care in accordance with standards of nursing practice and the written plan of care.
• Follow and implement individualized care plans developed by the Nurse Manager and interdisciplinary team.
• Perform medication administration and prescribed treatments per the plan of care and facility policies.
• Monitor residents’ conditions and promptly report changes to the supervising nurse or physician.
• Maintain accurate, timely documentation in accordance with orders, care plans, and regulatory standards.
• Support a safe, respectful, and therapeutic environment for residents and families.

Responsibilities for Licensed Practical Nurse (LPN):

• Provide patient care in accordance with accepted standards of nursing practice, the written plan of care, and the physician’s plan of treatment.
