Homestead of Abilene Memory Care in Abilene, KS is seeking an LPN to join our team, providing patient care under the supervision of the Director of Nursing. You will follow care plans, coordinate with the care team, and contribute to documentation and patient education.

Qualifications

• Active LPN license in Kansas.
• Experience in memory care or long-term care preferred.
• Ability to communicate with patient and family.

Responsibilities

• Provide care to patients per care plans and coordinate with physicians, patient, family, and staff as needed.
• Communicate with patient and family about education and progress.
• Review and maintain clinical records required for patient care and progress.
• Review weekly reports to monitor compliance with Kansas and Federal requirements (infection control, bloodborne pathogens, hazardous materials, fire safety).

As an LPN, you will work under the general supervision of the communities Director of Nursing to:

• Provide a provision of care to patients in accordance with their care plans. May require conferences with physician, patient, family members and other staff involved with patient’s care.
• Communicate with patient and family regarding education and progress of patient.
• Review and maintain all clinical records, which are required for patient’s care and progress.
• Review weekly reports to monitor personal and staff compliance to all Kansas and Federal requirements including blood borne pathogens, hazardous materials, infection control, and fire safety
