Job summary

RHA Health Services, LLC is seeking a Licensed Practical Nurse (IDD LPN) for day shifts in Rome, GA. This full-time onsite role supports individuals with intellectual and developmental disabilities in a group home setting.

The position involves holistic nursing, health data gathering, care coordination, and on-site clinics with a focus on teamwork with the interdisciplinary staff. The candidate must hold an active LPN license, CPR certification, and be able to work 7am–7pm with rotating

Qualifications

  • Graduation from an accredited licensed nurse program.
  • Current LPN license with no restrictions.
  • Current CPR Certification or eligibility to certify for CPR.
  • Ability to communicate effectively with Interdisciplinary Team Members, as well as the people supported and their families.
  • Ability to work independently, be organized, and prioritize tasks.

Responsibilities

  • Provide holistic nursing care to individuals with IDD.
  • Observe and report health needs to the registered nurse.
  • Coordinate preventative and routine medical care for people supported.
  • Document nursing care provided.
  • Assist physicians with on-site medical clinics.

No relocation packaged offered.Schedule: 7am - 7pm, rotating weekends offMax Patient Caseload: 4 totalJob Responsibilities:Provide holistic nursing care to individuals with intellectual and developmental disabilitiesObserve, assist with gathering health related data, and report the health needs or the people we support to the registered nurseCoordinate preventative and routine medical care for people supportedDocument nursing care providedAssist physicians with on-site medical clinics where applicableProvide health related trainings to unlicensed personnel according to state regulationsConduct medication pass observations in the home to ensure staff competency according to state regulationsConduct medication closest assessmentsPerform quality assurance checks and observations to ensure unlicensed personnel are performing health related tasks as assigned according to state regulationsObserve for and report any abnormal health related findings to the physicianCommunicate with physicians, pharmacies, medical suppliers, and the Interdisciplinary TeamAdvocate for the people supported and assist with developing community supports to promote optimal health for the people supportedMINIMUM QUALIFICATIONS:Graduation from an accredited licensed nurse program.Current LPN license with no restrictions to practice nursing.Current CPR Certification or eligibility to certify for CPR.Possess credentials required by Federal or State lawMust be able to communicate effectively with Interdisciplinary Team Members, as well as the people supported and their families.Must be able to work independently, be organized, and possess the ability to prioritize work tasks.Must be at least 18 years of agePHYSICAL DEMANDS AND WORK ENVIRONMENT:Must be able to regularly lift and carry up to 20 lbs. and occasionally pull at least 40 lbs.Must be able to squat, kneel, crawl, crouch, climb, and stoop as part of regular job duties.Must be able to use hands and fingers to handle or operate objects, tools, or controls. Required to stand and walk for extended periods.Must be able to demonstrate proficiency in CPR from floor level, which requires working on hands and knees, bending, standing, and lifting.Vision requirements include close, distance, and peripheral vision.Must be able to talk and hear.Exception – Deaf and Hard of Hearing Programs: For these programs only, employees must be fluent in American Sign Language (ASL). The requirement to talk and hear does not apply.Pre-employment screening:Complete criminal backgroundName checked in the registries.
