Job summary

RHA Health Services, LLC is hiring an LPN for days to support individuals with IDD in a structured office-based setting with travel to group homes. The role is full-time, daytime hours, with a starting pay around $32/hr and additional incentives.

Work includes routine visits to residential homes, medical clinics, and collaboration with an interdisciplinary team to ensure quality patient care. Candidates should have an LPN license in good standing and CPR certification, with strong communication

Qualifications

  • Graduation from an accredited licensed nurse program.
  • Current LPN license with no restrictions to practice nursing.
  • Current CPR Certification or eligibility to certify for CPR.
  • Must communicate effectively with Interdisciplinary Team Members and families.
  • Must be able to work independently, be organized, and prioritize tasks.

Responsibilities

  • Provide holistic nursing care to individuals with intellectual and developmental disabilities.
  • Observe and report health needs to the registered nurse.
  • Coordinate preventative and routine medical care for people supported.
  • Document nursing care provided.
  • Assist physicians with on-site medical clinics where applicable.
  • Provide health-related trainings to unlicensed personnel per state regulations.
  • Conduct medication pass observations to ensure staff competency per state rules.
  • Perform quality assurance checks to ensure tasks are performed correctly.
  • Report abnormal health findings to the physician and communicate with the care team.
  • Advocate for people supported and help develop community supports for health.

Location: Main office is out of Lumberton, NC and all canidates must be willing to travel to our 7 group homes. Employment Type: Full-time, Day Shift Compensation: $32/hr+ Incentives: No sign-on bonus is being offered at this time. No relocation packaged offered. Schedule: Monday - Friday, 8:30am - 5:00pm **This is an office-based position that supports our group home operations and requires routine visits to our residential homes. It is ideal for nurses seeking a more structured and steady-paced environment outside of traditional fast-paced hospital or SNF settings. Candidates with prior IDD and case management experience are strongly encouraged to apply.**

Job Responsibilities
  • Provide holistic nursing care to individuals with intellectual and developmental disabilities
  • Observe, assist with gathering health related data, and report the health needs or the people we support to the registered nurse
  • Coordinate preventative and routine medical care for people supported
  • Document nursing care provided
  • Assist physicians with on-site medical clinics where applicable
  • Provide health related trainings to unlicensed personnel according to state regulations
  • Conduct medication pass observations in the home to ensure staff competency according to state regulations
  • Conduct medication closest assessments
  • Perform quality assurance checks and observations to ensure unlicensed personnel are performing health related tasks as assigned according to state regulations
  • Observe for and report any abnormal health related findings to the physician
  • Communicate with physicians, pharmacies, medical suppliers, and the Interdisciplinary Team
  • Advocate for the people supported and assist with developing community supports to promote optimal health for the people supported
Job Requirements
  • Graduation from an accredited licensed nurse program
  • Current LPN license with no restrictions to practice nursing.
  • Current CPR Certification or eligibility to certify for CPR
  • Must be able to communicate effectively with Interdisciplinary Team Members, as well as the people supported and their families
  • Must be able to work independently, be organized, and possess the ability to prioritize work tasks
  • Physical requirements include: ability to regularly lift 20 pounds.
  • Must be able to lift a minimum of 40 pounds.
  • Must be able to pull a minimum of 25 pounds.
  • Must be able to squat, kneel, crawl, crouch, climb, and stoop.
  • Must be able to regularly stand and walk.
  • Must be able to demonstrate proficiency in CPR from the floor level.
  • Vision requirements include close vision, distance vision, and peripheral vision.
  • Must be able to communicate.

  • Complete criminal background Name checked in the registries. (OIG exclusions database, Child Abuse Registry, and Offenders Against Individuals with Developmental Disabilities)
  • Drug testing
  • Education verification and other credentialing based on position requirements.
  • Proof of employment history or references (if required)
  • Positions that require driving Proof of driver's license, driver's insurance, and vehicle, IF required for providing transportation for individuals.
