Licensed Practical Nurse (LPN) - PRN

Location: RHD, Schedule: FT | Varied Shifts

Pay Rate: $33.00 per hour

Position Summary

Abraxas Youth and Family Services is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to provide quality healthcare services to youth in a residential treatment environment. Working under the supervision of a physician or registered nurse, the LPN is responsible for supporting client health and wellness through direct care, medication administration, health education, documentation, and emergency medical response.

This role is ideal for a nursing professional who is passionate about making a meaningful impact while working as part of a multidisciplinary treatment team.

Key Responsibilities
  • Assist with the delivery of client medical care under the supervision of licensed medical professionals.
  • Serve as the designated Responsible Health Authority.
  • Administer prescribed medications and vaccinations in accordance with physician orders.
  • Monitor and maintain medication and medical supply inventories.
  • Coordinate referrals to physicians, specialists, and community healthcare providers.
  • Respond to medical emergencies and provide first aid as needed.
  • Participate in on-call coverage for emergency medical situations.
  • Maintain accurate medical records, health histories, and documentation in compliance with HIPAA regulations.
  • Participate in performance improvement and quality assurance initiatives.
  • Provide health education to clients, including hygiene, nutrition, and wellness practices.
  • Attend and contribute to multidisciplinary treatment team meetings.
  • Coordinate routine and emergency medical services for clients and staff.
  • Promote healthy habits and preventive healthcare practices.
  • Evaluate the quality and effectiveness of medical services provided.
  • Ensure compliance with applicable healthcare licensing and regulatory requirements.
Qualifications

Required

  • Current Licensed Practical Nurse (LPN) license or Registered Nurse (RN) license in the State of Illinois.

Additional Requirements
  • Must be at least 21 years of age.
  • Valid driver's license required.
  • Successful completion of physical examination, TB testing, and drug screening.
  • Ability to pass federal, state, and organizational background checks.
  • Satisfactory completion of employment verification, reference checks, and driving record review.
  • Ability to safely manage and respond to aggressive behavior when necessary.
