Job summary

Trilogy Home Healthcare Clearwater Office is seeking a Licensed Practical Nurse (LPN) to join our home health team on a PRN/per diem basis, including weekend coverage. You will provide compassionate care to patients in their homes, following the plan of care and coordinating with clinicians.

Active Florida LPN license and a valid driver's license are required. This role offers flexible scheduling, strong team support, and opportunities to grow within a Humana company recognized for quality home

• Must be a graduate of an accredited nursing program.
• Active LPN license in Florida is required.
• Valid Florida driver's license, vehicle insurance, and clean MVR.

What You Bring:

• Graduate of an accredited school of nursing
• Active LPN license in Florida
• Valid Florida drivers license, active vehicle insurance, and clear MVR
• Minimum 1 year nursing experience required; Home Health experience is a plus
• IV certification preferred
• Strong initiative and independent judgement
