Job summary

Visiting Nurse and Hospice for VT & NH is seeking an LPN to join our home health team in Bradford, VT. You will deliver patient care under RN supervision, support plans of care, and help patients stay safely at home.

Qualified candidates hold an active LPN license in VT and NH, with 1–2 years in home care preferred. You will document clearly, communicate effectively, and adapt to changing schedules to meet patient needs.

Qualifications

• Active LPN license and ability to work in VT and NH.
• 1-2 years’ experience in home care preferred.
• Ability to communicate clearly, verbally, and in writing.

Responsibilities

• Performs prescribed patient care to patients as delegated.
• Participates in patient case conferences as appropriate.
• Assists RN in the plan of care evaluation and modification.
• Observes and reports patient needs and reactions to treatment and medications.
• Provides nursing care with privacy, comfort and safety in the home.
• Demonstrates ability to instruct patients in home and community programs.
• Documents patient information clearly and within established time frames.

Responsibilities of a Licensed Practical Nurse include:

• Performs prescribed individualized patient care to patients as delegated.
• Participates in patient case conferences as appropriate.
• Assists the RN in the preparation, evaluation and modification of the patient's plan of care as appropriate.
• Observes and reports to the RN the patient's physical and emotional needs and reactions to treatment and to medications.
• Provides nursing care in a way that considers patient's privacy, comfort and safety.
• Demonstrates the ability to instruct patients in the home and at community programs.
• Performs procedures based on the Agency's policies and procedures.
• Performs selected procedures based on the Agency’s competency testing.
• Administers medication after successful completion of a medication competency.
• Accepts verbal orders from physicians.
• Assists the RN in planned patient teaching.
• Documents patient information in a clear, legible and accurate manner according to policy.
• Documentation is completed within established time frames to meet Federal and State requirements.

Qualifications of a Licensed Practical Nurse include:

• Must possess an active LPN license in both VT and NH.
• 1-2 years’ experience in home care preferred.
• Ability to communicate clearly, verbally, and in writing.
• Flexible in adapting to work schedule changes in order to meet patient’s 24 hour needs.
