Henry Ford Health seeks a Licensed Practical Nurse for Home Health Care/Hospice, based in Detroit. This full-time role provides adult and geriatric home health care under RN supervision, delivering technical and routine nursing services with interdisciplinary collaboration.

Qualifications include current LPN license, recent program completion, strong communication, problem-solving, and critical thinking skills, plus basic computer knowledge.

To provide adult and geriatric home health care with the supervision of the primary nurse that is collaborative with all members of the interdisciplinary team.

JOB SUMMARY:

Under the supervision of a RN or provider, the LPN uses sound nursing judgment in the provision of nursing care. Performs a variety of both technical and routine nursing services. Collaborates with health care teams to implement the nursing care plan. Demonstrates clinical competence.

EDUCATION/EXPERIENCE REQUIRED:

CERTIFICATIONS/LICENSURES REQUIRED:

• LPN License within 90 days of hire
