Job summary

Hartford HealthCare at Home is seeking a Revisit Licensed Practical Nurse (LPN) to provide skilled nursing care in clients\' homes and community settings under RN supervision. The role emphasizes outcome-based, cost-effective care with EMR documentation and collaboration with the care team.

Minimum requirements include a CT LPN certificate and a current LPN license and CT driver’s license. The position covers Torrington and surrounding towns with occasional weekend/holiday coverage.

Qualifications

• High School diploma or GED.
• CT state approved LPN certificate program is required.
• Experience with computer skills (Outlook, Word, Excel) is preferred.

Responsibilities

• Provide intermittent skilled nursing under the direction of an RN.
• Record daily progress in EMR.
• Reports to primary nurse, supervisor, physician, or social worker; communicates with the team.
• Collaborate with paraprofessional staff and other disciplines to ensure quality care.

Shift Detail: 8-4:30 Mon-Fri. Cover Torrington and surrounding towns. Cover 1 weekend a month, 1 major and 2 minor holidays a year.

As part of a person-centered care model, and under the direction of the RN, this role will provide skilled nursing care to clients in home and community-based settings ensuring the delivery of outcome based, high quality, cost-effective care. This role will collaborate with all team members to achieve exceptional outcomes.

Provide intermittent skilled nursing services in accordance with the established plan of care under the direction of an RN.

Provides support to patient/family utilizing teaching principles to promote and restore health and prevent disease and disability.

Records daily progress in EMR.

Reports to primary nurse, supervisor, physician, social worker, or therapist regarding cases.

Communicates/conferences with primary nurse and other disciplines.

Consults with physician regarding client needs and plan of care.

Assists in evaluation of care given by paraprofessional staff.

Participates on agency committees and special projects.

Qualifications

Minimum Requirements

• High School diploma or GED
• Successful completion of a CT state approved LPN certificate program

Experience

• Computer skills including but not limited to Outlook, Microsoft Word, and Excel

Licensure

• Current and valid LPN license
• Valid CT driver’s license & a good driving history
