Base pay range: $44,600.00/yr - $61,400.00/yr

Job Description

Make a meaningful impact every day as a CenterWell Home Health nurse, providing personalized, one-on-one care to help patients regain independence in their homes. Collaborate with physicians and clinicians to develop and manage care plans supporting recovery and improving quality of life.

Responsibilities

• Provide professional nursing services under RN supervision, compliant with all applicable laws, standards, and policies.
• Deliver skilled nursing interventions for treatment, rehabilitation, and prevention, using a holistic approach.
• Apply clinical knowledge to facilitate problem resolution and achieve patient outcomes.
• Coordinate with RNs and physicians regarding care plans and orders.
• Implement and document the nursing plan of care, maintaining confidentiality and professional standards.
• Communicate effectively with patients, families, and healthcare team members.
• Participate in care coordination and discharge planning as needed.

Minimum Requirements

• Graduate of an accredited LPN program.
• Current nursing license in the practicing state.
• Valid driver’s license, auto insurance, and reliable transportation.
• Current CPR certification.
• At least two years of clinical experience as an LPN/LVN, preferably in home health or hospice.

Additional Details

Scheduled weekly hours: 40

Salary details include a per visit/unit pay range of $28.00 - $40.00, with an annual base pay of $44,600 - $61,400, depending on location and experience.
