Job Description

  1. Provide professional nursing services under the direction of a RN in compliance with the state's Nurse Practice Act, any applicable licensure/certification requirement, applicable healthcare standards, governmental laws and regulations, and CenterWell Home Health policies and procedures.
  2. Provide skilled nursing interventions in the treatment of the patient/clients illness, rehabilitative needs and preventative care. Utilize a holistic approach in the provision of problem specific therapeutic interventions, teaching and training activities in accordance with the plan of care.
  3. Apply knowledge and skills in accordance with accepted standards of clinical practice to facilitate problem resolution and achieve individualized patient goals and outcomes.
  4. Confer with the RN Supervisor regarding needed changes in the Plan of Treatment. Accept verbal orders from physician where permitted by law/regulations/Nurse Practice Act and communicate these orders to the RN Supervisor.
  5. Utilize a systematic, individualized goal/outcome driven approach in implementing the nursing plan of care.
  6. Maintain contact with patients, physicians, clinical managers, and other healthcare team members regarding patient/family needs and status changes. Participate in care coordination and discharge planning as appropriate.
  7. Maintain the highest standards of professional conduct regarding confidential information. Share information only when authorized and in the best interests of the patient.
  8. Communicate effectively to ensure adherence to professional standards and availability of supplies, materials, and equipment needed for safe and effective care.
  9. Prepare, submit, and maintain documentation as required by the Company and/or facility. Document visits/shift notes on the day services are rendered.

As a Home Health LPN, You Will

Required Experience/Skills

  1. Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
  2. Current nursing license in the practicing state.
  3. Valid driver’s license, auto insurance, and reliable transportation.
  4. Current CPR certification.
  5. Two years experience as an LPN/LVN in a clinical setting, preferably in home health or hospice.

Pay Range

The compensation range below reflects an estimate of starting pay for full-time employment at the time of posting. Pay may vary based on location and individual qualifications.

  • $28.00 - $40.00 per visit/unit
  • $44,600 - $61,400 per year base pay
