As a Home Health LPN, You Will:

• Provide professional nursing services under the direction of a RN in compliance with the state's Nurse Practice Act, any applicable licensure/certification requirements, healthcare standards, laws, and policies.
• Provide skilled nursing interventions in the treatment of patient/clients' illnesses, rehabilitative needs, and preventative care, using a holistic approach.
• Apply knowledge and skills according to accepted standards to facilitate problem resolution and achieve patient goals.
• Confer with the RN Supervisor regarding changes in the Plan of Treatment and communicate orders as permitted by law.
• Implement the nursing plan of care with a goal/outcome-driven approach.
• Maintain contact with patients, physicians, and healthcare team members regarding patient needs and status changes.
• Maintain confidentiality and share information only when appropriate.
• Ensure availability of supplies and equipment needed for care.
• Prepare and submit documentation as required, including visit/shift notes.

Required Experience/Skills:

• Graduate of an accredited LPN/Vocational Nursing program.
• Current nursing license in practicing state.
• Valid driver’s license, auto insurance, and reliable transportation.
• Current CPR certification.
• At least two years of clinical experience as an LPN/LVN, preferably in home health or hospice.

Pay Range:

• $28.00 - $40.00 per visit/unit
• $44,600 - $61,400 per year (base pay)
