Job summary

The Visiting Nurse Association of Hanover & Spring Grove, Inc. is hiring a Licensed Practical Nurse (LPN) for its Home Health team in Pennsylvania. This role focuses on providing compassionate, one-on-one nursing care in patients' homes under an RN's supervision.

You’ll administer medications, educate patients and caregivers, monitor progress, and help patients stay safe and independent at home. A Pennsylvania LPN license, valid driver's license, reliable transportation, and a patient-centered

Responsibilities

• Deliver safe, high-quality nursing care under RN guidance.

• Assist with activities of daily living and promote independence.
• Observe for changes and notify RN Case Manager.
• Maintain timely, accurate documentation.
• Coordinate care with the interdisciplinary team.
• Provide visits to Hospice patients as needed.

What You'll Do:

As a Home Health LPN, you'll play an important role in helping patients remain safe, comfortable, and independent at home.

• Under the direction of an RN, deliverssafe, high-quality nursing care as outlined in the patient’s plan of care.
• Administer medications and treatmentsin accordance withthe PA Nurse Practice Act.
• Monitor and report patient responses to medications and treatments.
• Assess and educate patients and caregivers on disease management, medications, and self-care techniques.
• Assistpatients with activities of daily living, promoting independence and comfort.
• Observe forchanges in condition and report to RN Case Manager or Clinical Manager.
• Maintaintimelyandaccuratedocumentation of all care provided.
• Participate in coordination of care with the interdisciplinary team to ensure continuity.
• Provide visits to Hospice patients as needed.
