The Visiting Nurse Association of Hanover & Spring Grove is hiring a Licensed Practical Nurse (LPN) for our Home Health team serving Adams and York Counties in Pennsylvania. Join a caring organization dedicated to delivering compassionate, patient-centered care in the home setting.

As an LPN, you will work under an RN, administer medications, assess patients, educate families, and help patients stay safe and independent at home.

Qualifications

• Pennsylvania LPN license valid.
• Compassionate heart and patient-focused.
• Strong communication and organizational skills.
• Ability to work independently while being part of a team.
• Valid driver’s license and reliable transportation.
• Home health experience is a plus.

Responsabilités

• Provide safe, high-quality nursing care per plan of care.
• Administer medications and treatments per PA Nurse Practice Act.
• Monitor and report patient responses.
• Educate patients and caregivers on disease management.
• Assist with activities of daily living to promote independence.
• Observe changes in condition and report to RN.
• Maintain timely and accurate documentation of care.
• Coordinate care with the interdisciplinary team for continuity.
• Provide Hospice visits as needed.

What You'll Do:

As a Home Health LPN, you'll play an important role in helping patients remain safe, comfortable, and independent at home.

• Under the direction of an RN, deliverssafe, high-quality nursing care as outlined in the patient’s plan of care.
• Administer medications and treatmentsin accordance withthe PA Nurse Practice Act.
• Monitor and report patient responses to medications and treatments.
• Assess and educate patients and caregivers on disease management, medications, and self-care techniques.
• Assistpatients with activities of daily living, promoting independence and comfort.
• Observe forchanges in condition and report to RN Case Manager or Clinical Manager.
• Maintaintimelyandaccuratedocumentation of all care provided.
• Participate in coordination of care with the interdisciplinary team to ensure continuity.
• Provide visits to Hospice patients as needed.

What We're Looking For:
