Responsibilities

The Licensed Practical Nurse (LPN) in Home Health provides quality patient care and performs technical skilled care in the patient's home.

• Provides services within the scope of practice defined by state laws, in accordance with the plan of care, and in coordination with the healthcare team. May not alter the plan of care.
• Completes and submits a progress note for each visit daily.
• Observes and reports the patient's nursing needs. Instructs family members in patient care and maintaining a healthy environment.
• Records and reports to the supervising nurse and/or physician the patient's response to treatment and any changes in condition.

Education and Experience

License Requirements

• Current LPN licensure in the state of practice
• Valid Driver's License, vehicle insurance, and access to reliable transportation
• CPR Certification required

Additional State-Specific Requirements

• CA: One year of professional nursing experience as an LPN
• LA: Current unrestricted LPN Licensure in LA and at least one year of LPN experience
• Other states: No additional requirements
