Job summary

Pinnacle Home Care in Florida's Deland area is hiring a Licensed Practical Nurse to provide in-home skilled nursing under RN supervision. You will use clinical expertise to support patient care plans, document visits, and coordinate with the care team.

Ideal candidates have an LPN/LVN license, BLS, 1 year of home health experience, strong communication and time management, and a valid driver’s license with reliable vehicle to travel to patient homes.

Qualifications

• Current LPN or LVN license (state-specific).
• Graduate of an accredited nursing program.
• Valid BLS certification.
• At least 1 year nursing experience (home health or medical/surgical preferred).
• Excellent communication, organizational, and time-management skills.
• Valid driver’s license and access to a reliable, insured vehicle.
• Ability to visit homes that may not be wheelchair accessible.
• Adequate hearing, eyesight, and dexterity for physical assessments.
• Able to perform walking, standing, stooping, and occasional lifting or turning of patients.

Responsibilities

Key Responsibilities

• Complete visit reports promptly and accurately within the electronic medical record system to support high-quality care and regulatory compliance, including all required documentation.
• Confirm daily visit schedules to ensure seamless coordination with other staff.
• Demonstrate awareness of the agency’s Quality Assurance and Performance Improvement (QAPI) program and participate as required.
• Understand and adhere to the agency’s process for managing and addressing customer complaints.
• Maintain acute awareness of patient rights, ensuring these rights are upheld in accordance with the agency’s service agreements and admission policies.
• Understand the agency’s policies regarding appropriate patient discharge and reasons for discharge.
