UPMC Home Health is hiring a full-time LPN for the Mount Washington, Greentree, Crafton, Corliss, Carnegie, and surrounding areas.

Now offering a sign-on bonus of up to $12,500 for experienced LPNs!

Act as a healthcare team member by exercising sound nursing judgment based on preparation, knowledge, experience, and competency. Provide care following UPMC LPN Practice Guidelines, policies, and State Board Regulations. The LPN participates in planning, implementing, and evaluating nursing care, often under the supervision of a Registered Nurse or other licensed provider.

Responsibilities:

1. The LPN functions as a healthcare team member, exercising sound nursing judgment and participating in care planning, implementation, and evaluation.
2. Communicates with licensed professionals and team members when patient care needs exceed the LPN's scope or expertise, or if patient's condition deteriorates.
3. Follows written policies, procedures, and guidelines. Formulates daily goals and care plans involving the patient as a partner, and communicates patient conditions to other providers.
4. Ensures comprehensive documentation to promote communication and improve patient safety and satisfaction. Builds caring relationships with patients, families, and colleagues. Participates in unit activities and assists team members as needed.
5. Provides patient and family education, seeks feedback, and engages in continuous professional development. Identifies opportunities for quality and workplace improvements.
6. Develops productive relationships, demonstrates accountability, and respects cultural differences. Maintains work-life balance and reports safety hazards.

Qualifications:

• Graduation from an approved Licensed Practical Nursing Program.
• Experience in healthcare preferred but not required.
• Physical stamina for walking, standing, lifting, and positioning patients.
• Computer skills for electronic health records.

Licensure, Certifications, and Clearances:

• Maintains valid state licensure or multistate license if applicable.
• Completes licensure exam within four months of hire or one year of graduation.
• Required certifications include BLS or CPR, Driver's License, and background clearances (Act 33, Act 34, OAPSA).
