Licensed Practical Nurse - Geriatrics and Extended Care

Summary

This LPN position is located within the Grand Island Community Living Center in Grand Island, Nebraska, providing care to Veterans receiving extended hospital care and short-term rehabilitation, including assistance with activities of daily living, skilled nursing, and dementia care. The mission of the Community Living Center is to restore each Veteran to their highest level of well-being, prevent health declines, and provide end-of-life comfort.

Duties
  • Perform assessments and interventions in health promotion, wound care, physical/psychological/social assessments, support care plans, palliative care, and hospice.
  • Prioritize patient needs, support care plans, and communicate changes in patient condition to RN or providers.
  • Prepare and administer medications as prescribed.
  • Perform complex procedures such as catheterizations, venipuncture, dressing changes, and flushes.
  • Assist with diagnostic tests and procedures.
  • Collect specimen samples.
  • Support suicide prevention initiatives.
  • Respond to emergencies and initiate life-saving measures.
  • Document assessments and care data into electronic medical records.
  • Mentor lower-grade LPNs and unlicensed staff.
  • Other duties as assigned.
Work Schedule

Evening (2:00pm - 2:00am) and Night Shift (6:00pm - 6:30am), rotating weekends and holidays. Flexible scheduling available.

Compensation and Benefits

Requirements
  • U.S. citizen; VA employment testing and background checks required.
  • Proficiency in English; BLS certification required (training provided).
  • Must pass physical exam and participate in vaccination program.
Qualifications

Minimum education and licensure as a practical/vocational nurse, with specific experience requirements for GS-3 to GS-6 levels. Full licensure in a U.S. jurisdiction required. Experience must demonstrate KSAs such as patient care, medication administration, emergency response, and teamwork.
