Heritage Health Services in Sturgeon Bay, Wisconsin is seeking a Licensed Practical Nurse (LPN) to join our resident-centered care team. Under supervision, you will assess residents, implement plans of care, and evaluate outcomes, delivering compassionate nursing care in a supportive environment.

The role requires an active LPN license and CPR certification, strong communication, and the ability to calculate dosages precisely. We welcome experienced nurses and new grads with training support.

Summary:

Under the supervision of the Unit Manager/ADON/DON, a Licensed Practical Nurse/Licensed Vocational Nurse assumes responsibility and accountability for a group of residents/patients during a shift. Nursing care is provided through assessment, implementation, and evaluation of the plan of care.

Who We're Looking For

• Active LPN license and current CPR certification

• Strong communication skills, basic computer proficiency and ability to meet all legal health requirements
