Job summary

Elizabeth Care Center in Elizabeth, WV is hiring a compassionate Licensed Practical Nurse (LPN) to join our clinical team. The LPN administers medications, monitors resident conditions, and collaborates with the interdisciplinary team to support positive resident outcomes.

Position Summary

We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our clinical team. The LPN provides high-quality nursing care, administers medications and treatments, monitors resident conditions, and collaborates with the interdisciplinary team to support positive resident outcomes.

Key Responsibilities

• Administer medications and treatments as ordered.
• Monitor residents and promptly communicate changes in condition.
• Provide direct nursing care and support activities of daily living as needed.
• Collaborate with physicians, nursing staff, and the interdisciplinary team on resident care plans.
• Maintain accurate and timely clinical documentation.
• Communicate effectively with residents, families, and healthcare providers.
• Promote a safe, respectful, and resident-centered care environment.

Qualifications & Requirements

• Current, unencumbered West Virginia LPN license.
• Skilled nursing or long-term care experience preferred.
• Strong clinical judgment, communication, and organizational skills.
• Ability to work independently and collaboratively as part of a care team.
• Compassionate and resident-focused approach to care.
• Ability to meet the physical requirements of the position, including lifting/moving 50 pounds or more and standing or walking for extended periods.
