Job summary

Elizabeth Park Rehabilitation and Nursing LLC is seeking a compassionate Licensed Practical Nurse (LPN) for a 7:00 AM to 3:30 PM shift in New Jersey. You will provide direct patient care, administer medications, and help develop care plans in collaboration with the healthcare team.

Ideal candidates hold an active NJ LPN license, with 1+ year in long-term care or rehabilitation, and demonstrate strong communication and empathy for patients and their families.

Qualifications

• Current New Jersey LPN license.
• 1+ year experience in long-term care or rehabilitation.
• Ability to lift, transfer, and reposition patients.
• Strong communication with patients, families and healthcare teams.

Responsibilities

• Provide direct patient care including medications, wound care, and treatments as prescribed.
• Develop and implement individualized care plans with patients and families.
• Monitor patient progress and suggest care-plan modifications.
• Collaborate with RNs, PTs, OTs, and social workers for comprehensive care.
• Participate in patient and family education and ensure clear communication.
• Maintain accurate records in accordance with standards.

Responsibilities:

• Provide direct care to patients and residents, including administering medications, wound care, and other treatments as prescribed by physicians
• Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams
• Monitor and report patient progress, identifying areas for improvement and making recommendations for modifications to care plans
• Collaborate with other healthcare professionals, including RNs, PTs, OTs, and social workers, to ensure comprehensive care
• Participate in patient and family education, promoting effective communication and empowerment
• Maintain accurate and detailed records, adhering to regulatory and facility standards

Requirements:

• Current LPN licensure in the state of New Jersey
• High school diploma or equivalent required; ADN or BSN degree preferred
• 1+ year of experience as an LPN in a long-term care or rehabilitation setting; experience in rehabilitation and nursing highly valued
• Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams
• Ability to lift, transfer, and reposition patients, as well as perform other physical demands of the role
