Qualifications

• Graduate from an accredited nursing program and obtain state LPN licensure.
• Minimum of 1 year of long-term care experience, or an equivalent combination of education and experience.
• Must maintain CPR/First-Aid certification.

Responsibilities

• Provide direct resident care and treatments; assess needs and modify plans.
• Administer medications and document administration in accordance with policy.
• Monitor and record vitals and changes in residents’ conditions.
• Prepare paperwork for laboratory requests and collect samples as needed.
• Assist with treatments, wound care, and comfort measures.
• Document nursing history and assessments; maintain confidentiality and HIPAA compliance.
• Respond to call-lights and coordinate with the Wellness Director as needed.
• Attend required trainings and meetings and perform other duties as assigned.
