Responsibilities

• Oversee daily resident care needs and coordinate care plans.
• Communicate changes in residents’ conditions clearly to residents, families, and the care team.
• Collaborate with Medical Director and interdisciplinary team for individualized care plans.
• Provide guidance and support to nursing assistants and caregiving staff.
• Monitor resident health and respond promptly to changes in condition.
• Foster meaningful relationships with residents and families to promote trust.
