Licensed Practical Nurse - ACT Team

Location: New York, NY (Onsite)

Employment Type: Contract, 20 hours per week

Pay Range: $30.87-$44.10 Per Hr

Description: Provide nursing services as a member of a mobile multi-disciplinary treatment team. Support individuals with mental illness in securing permanent housing and residential support. Offer psychiatric treatment and individual/group substance abuse treatments. Conduct daily living skills training and financial management skills.

Key Responsibilities

• Conduct physical health assessments.
• Assess health needs and refer participants to community-based medical care.
• Manage and administer intramuscular injections and supply medications as prescribed.
• Provide treatment, rehabilitation, and support services.
• Meet regularly with participants to assess health status.
• Provide health management education.
• Communicate with primary medical providers to coordinate treatment.

Qualifications

• Licensed LPN within the state of New York.
• Excellent verbal, written, and interpersonal communication skills.
• Proficient in generic word processing and spreadsheet software.
• Ability to research material via the internet.
