Responsibilities

• Complete and file admission, transfer, and discharge forms; document treatment plans and orders.
• Chart nursing notes accurately reflecting care provided and resident response.
• Administer medications as ordered and verify resident identity.
• Monitor residents for changes in condition and communicate with physicians.
• Participate in quality assurance and coordination of resident care plans.
