Licensed Practical Nurse Care Manager / Case Management (LPN)

Join us at HOPE Center as a Licensed Practical Nurse (LPN) in Bronx, NY.

Salary: $31.94 per hour, with an additional $0.25 shift differential for evenings and nights.

Responsibilities

• Collecting information from new Residents for admission.
• Recording health details, including vitals and temperature.
• Administering medications and injections as needed.
• Treating and dressing wounds and bedsores.
• Supervising Certified Nursing Assistants (CNAs) may be required.
• Assisting Residents with dressing and personal hygiene.
• Monitoring food and liquid intake/output of Residents.

Requirements

• Ability to work as part of a team.
• Valid LPN license in the state.
• Good standing with State Registry.
