• Optimizes care coordination with hospitals, emergency room, consulting physicians, and community resources as needed.
• Develops workflow to ensure smooth transition of care for patients treated in facility by a specialty position or other healthcare provider.
• Reviews after visit summary with patients when appropriate.
• Triage patients to ensure productive departmental flow.
