Concordia Visiting Nurses in Western Pennsylvania seeks a dedicated Licensed Practical Nurse (LPN) to join our home health team. You will provide skilled nursing care, monitor patients, and support families in their homes.

What you’ll do as a Licensed Practical Nurse LPN:

As a Home Health LPN, a vital part of the patient's care team is provided by the employee, offering skilled nursing care and support. This includes tasks such as:

• Monitoring vital signs
• Assisting with wound care and personal care
• Educating patients and families
• Maintaining medical records
• Collaborating with other healthcare professionals. Also, provide emotional support and companionship, building genuine connections with patients.

Who you are as a Licensed Practical Nurse LPN:
