Job summary

Genesis Home Care is seeking a dedicated LPN to provide skilled nursing in patients' homes across South Zanesville, OH. You will follow physician orders, monitor patient conditions, and educate families to promote safe, effective care.

Ideal candidates have an active OH LPN license with 1–2 years acute care experience, and a compassionate, team-oriented approach. Valid driver's license required for travel within the service area.

Qualifications

• ActiveOhio LPN license required.
• Minimum 1 year acute care LPN experience; 2 years preferred.
• Home health experience preferred.
• Strong communication and interpersonal skills.
• Compassionate, patient-focused approach.
• Ability to work independently and prioritize duties.
• Reliable, detail-oriented with a team orientation.

Responsibilities

• Provide skilled nursing care per physician orders and RN plans of care.
• Assess patient conditions and report changes to RN Case Manager.
• Administer medications and treatments as prescribed.
• Educate patients and families on disease management and safety.
• Involve patients/families in the care plan.
• Complete accurate clinical documentation per Medicare requirements.
• Coordinate with care team and scheduler on patient needs and schedule changes.
• Participate in on-call coverage and meetings.
• Provide care within LPN scope and state/federal regs.
• Travel to patients' homes within the service area.

Schedule: Monday-Friday | 8:00 AM-5:00 PM (with on call requirements)

What You'll Do

• Provide skilled nursing care according to physician orders and RN-developed plans of care
• Assess and monitor patient conditions and communicate changes to the RNCM
• Administer medications and treatments as prescribed
• Educate patients and families on disease management, medications, safety, and preventative care
• Involve patients and families in their plan of care
• Complete accurate and timely clinical documentation in accordance with agency and Medicare requirements
• Communicate with the care team and scheduler regarding patient needs and schedule changes
• Participate in on-call coverage, case conferences, in-service training, and required meetings
• Provide care within the LPN scope of practice and applicable state and federal regulations
• Travel to patients' homes within the assigned geographic area

What We're Looking For

• Current, active LPN license in the state of Ohio
• Minimum of 1 year of acute care LPN experience required; 2 years preferred
• Home health experience preferred
• Strong communication and interpersonal skills
• Compassionate, patient-focused approach
• Ability to work independently, prioritize responsibilities, and make appropriate nursing judgments
• Strong attention to detail and reliable work ethic
• Ability to work effectively as part of a team
• Valid driver's license and auto insurance with your name listed as a driver

Education & Credentials

• Graduate of an approved practical nursing program
• Current, active LPN license in the state of Ohio
