Job summary

Oak Park Place is seeking a Licensed Practical Nurse (LPN) to provide thoughtful care to residents in a part-time, non-exempt role. You will follow each resident's Plan of Care, monitor health changes, and promptly update the care team on evolving needs.

Ideal candidates have prior nursing or medical field experience, strong communication, and the ability to thrive in a fast-paced setting while maintaining compassionate, patient-centered care.

Responsibilities

The Licensed Practical Nurse (LPN) is responsible for providing thoughtful care to residents and their families based on the personalized Plan of Care, monitoring and tracking changes in conditions and needs, and proactively updating the appropriate contacts on the plan of care, health changes, and concerns.
