Boost Home Healthcare is seeking a dedicated Licensed Practical Nurse to deliver skilled nursing care in patients' homes across Warrenton, VA.

You will implement the care plan, provide treatments, teach patients and families, and maintain accurate records while adhering to physician orders and policies. Flexible scheduling and ongoing development support your growth.

Qualifications

• Graduate of an approved school of practical nursing; licensed in the state of posting.
• Minimum of one year of LVN experience in home health care.
• Current BLS certification required.

Responsibilities

• Implement the nursing care plan for each patient.
• Provide nursing services, treatments and diagnostic and preventative procedures as assigned.
• Initiate preventive and rehabilitative nursing procedures as appropriate.
• Observe signs and symptoms and report to the physician and RN changes in condition.
• Teach and counsel the patient and family regarding care needs at home.
• Maintain accurate records of observations, treatments, and care.

What you'll be doing:

• Implement the nursing care plan for each patient.
• Provide nursing services, treatments and diagnostic and preventative procedures as assigned.

• Maintain accurate and complete records of observations, treatments, and care of patients.
