Job summary

Ohana Pacific Health's Home Health team is seeking a compassionate Licensed Practical Nurse for a full-time role delivering skilled nursing care in patients' homes. You will assess needs, coordinate services, and support patients and families in a home setting.

This position includes a $1,000 retention bonus, generous benefits, PTO, and opportunities for growth within Hawaii's largest locally owned post-acute health system.

Qualifications

• Graduate of an accredited school of nursing.
• Current Hawaii LPN license.
• Minimum one year recent healthcare experience.
• Current BLS certification.
• Current medical/health screening includes 2 step TB clearance.

Responsibilities

• Owns an assigned caseload of home health patients, assessing needs and coordinating services.
• May supervise LPNs and HHAs in daily tasks.
• Responds to patient, caregiver and staff inquiries.

• Hale Makua Health Services Home Health is a Medicare-certified provider of personalized care in the comfort of your home for individuals who are home-bound and need skilled help with medical conditions.

The Position

We're looking for passionate Licensed Practical Nurse who provides professional nursing care to patients following established policies and procedures of the agency.

• Assumes overall responsibility for an assigned case load of home health patients, providing assessment, coordination of ancillary services and community services, and may provide direct nursing health to the patient in the patient's place of residence.
• May oversee and direct Licensed Practical Nurses (LPNs) Home Health Aides (HHAs) in their day-to-day assignments.
• Answers questions from patients, responsible parties and other personnel.

Requirements

• Graduate of an accredited school of nursing.
• Current license as a License Practical Nurse in the State of Hawaii required.
• Minimum one-year recent experience in healthcare.
• Current BLS certification
• Current medical/health screening includes 2 step TB clearance
