Job summary

Amedisys, Inc. is seeking a licensed practical nurse to join our home health team in Vidalia, GA. This full-time position offers mileage reimbursement and flexible scheduling to support patients in their homes.

You will verify plans of care, collaborate with physicians and RNs, and teach self-care techniques to patients and caregivers. We offer competitive pay ($31–$36 per visit converting to base), PTO, HSA options, wellness rewards, mental health support, a 401(k) with company match, family

Qualifications

• One year of clinical LPN experience.
• Current LPN license, state(s) you are assigned to work.
• Current CPR certification.
• Valid driver's license with reliable transportation and liability insurance.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN frequently to ensure patient needs are met; Notify them as soon as possible when patient condition changes.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review all medications and perform a medication reconciliation at each visit.
• Perform specialized procedures ordered by physician (i.e. foley catheter change).
• Prepare equipment and materials for treatments using the aseptic technique.
• Complete documentation timely and accurately.
