Heritage Hall Healthcare & Rehab Centers in Virginia seeks a dedicated Licensed Practical Nurse (LPN) to join our compassionate team. You will provide direct care to residents, ensuring comfort, safety, and quality of life.

The role supports ongoing collaboration with the care team to uphold our standards of patient-centered care. The ideal candidate has graduated from an accredited nursing program, holds an active LPN license in Virginia, and values respectful, person-centered care.

Qualifications

• Must have graduated from an accredited nursing program.
• Must have an active LPN license in the state of Virginia.
• Experience in long-term care preferred.
• Ability to communicate effectively with a variety of individuals.
• Knowledge of nursing practices and procedures.
• Knowledge of laws, regulations, and guidelines governing nursing in a long-term care facility.
• Ability to make independent decisions.

Responsibilities

• Provide direct care to residents as part of the care team.
• Support and monitor residents' daily health needs.
• Collaborate with registered nurses, physicians, and other staff.
• Document care provided and report changes in residents' condition.
