Qualifications

  • Current Pennsylvania Licensed Practical Nurse (LPN) license in good standing.
  • CPR and First Aid certification current.
  • Valid driver's license with an acceptable driving record.
  • Pennsylvania Act 33 and Act 34 clearances.
  • FBI clearance, when applicable.
  • Completion of all training requirements associated with licensure and agency standards.
  • Successful completion of a physical examination, TB test, and Hepatitis B titer.

Responsibilities

  • Maintain nursing documentation and electronic health records (eMAR).
  • Conduct face-to-face and telephone interactions with participants.
  • Complete nursing assessments within 24 hours of admission.
  • Coordinate refills of non-controlled medications with the program psychiatrist.
  • Facilitate residential telepsychiatry appointments.
  • Educate participants regarding medications and treatment plans.
  • Monitor medication effects, side effects, and adverse reactions.
  • Communicate with nursing staff, program staff, physicians, and other healthcare providers.
  • Report health concerns to Nurse Manager and Program Director.
  • Perform medical mobile services and mobile crisis interventions as required.
  • Provide health education and wellness support to participants.
  • Maintain confidentiality per HIPAA and agency policies.

LICENSED PRACTICAL NURSE

Position Purpose

The Licensed Practical Nurse (LPN) is responsible for providing comprehensive nursing oversight, coordination, and direct care to individuals served within the program. This role supports all health-related needs through assessment, intervention, monitoring, education, and collaboration with interdisciplinary team members, family members, and healthcare providers. The LPN plays a vital role in promoting the physical and emotional well-being of participants while ensuring compliance with healthcare regulations and agency standards.

Major Responsibilities
  • Maintain accurate nursing documentation, medication records, and electronic health records.
  • Conduct face-to-face and telephone interactions with program participants as needed.
  • Complete nursing assessments within 24 hours of a participant's admission to the residence.
  • Coordinate refills of non-controlled medications in consultation with the program psychiatrist.
  • Facilitate and support residential telepsychiatry appointments through virtual platforms.
  • Educate participants regarding their medications and treatment plans.
  • Ensure medications are administered as prescribed and documented in the electronic Medication Administration Record (eMAR).
  • Monitor participants for medication effectiveness, side effects, and adverse reactions.
  • Communicate effectively with nursing staff, program staff, physicians, and other healthcare providers.
  • Report significant health concerns and incidents to the Nurse Manager and Program Director.
  • Conduct and complete medical mobile services and mobile crisis interventions in accordance with applicable regulations and program requirements.
  • Provide health education and wellness support through individual and group interventions.
  • Maintain strict confidentiality of participant information in accordance with HIPAA and agency policies.
  • Participate in interdisciplinary team meetings and care planning activities.
  • Perform other duties as assigned.
Qualifications
Required
  • Current Pennsylvania Licensed Practical Nurse (LPN) license in good standing.
  • Current CPR and First Aid certification.

  • Experience working with individuals receiving behavioral health or community-based services.
  • Strong communication, organizational, and documentation skills.
  • Ability to work independently and collaboratively within a multidisciplinary team.

  • Monitor and respond appropriately to changes in participant conditions.
Medication Administration
  • Administer medications safely and accurately.
  • Monitor for side effects, adverse reactions, and treatment effectiveness.
  • Ensure compliance with physician orders and medication protocols.
  • Work closely with physicians, psychiatrists, behavioral health professionals, and program staff to coordinate care and support participant wellness.
Documentation
  • Maintain complete, accurate, and timely electronic health records, including assessments, medication records, and nursing notes.
  • Educate participants regarding health conditions, medications, treatment options, and self-care practices to promote recovery, independence, and overall wellness.
Equipment and Technology
  • Proficient use of computers, tablets, mobile devices, and agency software systems.
  • Electronic Medication Administration Record (eMAR) systems.
  • Telehealth and virtual communication platforms.
  • Company-issued equipment, including vehicles when required.
Physical Demands

Medium Work: Exerting up to 50 pounds of force occasionally, up to 20 pounds of force frequently, and up to 10 pounds of force constantly to move objects.
