LICENSED PRACTICAL NURSE

Part‑time LPN | Wednesday – Friday 3 pm‑11 pm | 24 hours/week

Position Summary

The primary role of a Licensed Practical Nurse (LPN) is to provide comprehensive oversight, coordination, and hands‑on care in addressing all health‑related matters affecting program participants. This includes both direct and indirect health concerns, ensuring the physical and emotional well‑being of individuals through proactive assessment, timely intervention, and ongoing collaboration with interdisciplinary team members, families, and external healthcare providers. The LPN plays a critical role in maintaining a safe, responsive, and supportive environment that promotes optimal health outcomes for all individuals involved in the program.

Major Responsibilities
  • Maintains nursing documentation and medication records.
  • Assists residents in learning about their medications; assures that medications are taken as physician ordered and recorded in electronic MAR.
  • Provides nursing care including, but not limited to, wound care, trach care, diabetic counseling, etc.
  • Assesses residents’ health and emotional needs.
  • Communicates with other members of the nursing and program staff; communicates serious issues to Nurse Manager and Program Director.
  • Maintains confidentiality, including keeping voice low and not talking about residents in public areas.
  • Other duties as assigned.
Job Qualifications
  • Current PA RN/LPN licensure.
  • Current CPR/First Aid certification.
  • Valid Driver’s License.
  • Act 33/34 clearances.
  • FBI clearance as needed.
  • All trainings required by licensure.
  • Must complete a Physical, TB test, and Hep B titer.
Network Functions

Patient Care: LPN’s assess patient health, and implement nursing interventions to ensure optimal patient outcomes.

Medication Administration: Administer medications and monitor patients for side effects and reactions, ensuring adherence to prescribed treatment plans.

Collaboration: Work closely with physicians and other healthcare professionals to coordinate patient care and communicate updates regarding patient conditions.

Documentation: Maintain accurate and complete electronic patient records, documenting vital signs, symptoms, and changes in patient conditions.

Patient Education: Educate patients about health conditions, treatment options, and self‑care strategies to promote recovery and wellness through individual and group sessions.

Proficient use of computers, mobile devices, and agency software. Business use of company equipment including vehicle.

Requirements
Physical Requirements:
