Qualifications

• LPN with active license.
• 3-5 years helping patients manage chronic conditions (Required).
• 2-5 years care management experience with SDoH and behavioral health.
• Experience and knowledge of patient education methods (Required).
• Effective telephone interaction skills to engage patients successfully.
• EMR and Patient Management Platform use and experience (Required).

Responsibilities

• Engage patients telephonically with an interdisciplinary care team.
• Perform assessments and identify problems affecting health and wellness.
• Assist patients telephonically to create and implement person-centered care plans.
• Ensure compliance with clinical documentation requirements.
• Complete medication reconciliation and coordinate with specialists and other providers.
• Coordinate with the primary care physician and specialists for referrals and scheduling.
• Support patient resources to achieve health outcomes.
• Lead interdisciplinary care team conferences.
